How we protect your privacy

How will we keep your information private?

The National Psoriasis Foundation and the National Psoriasis Victor Henschel BioBank are committed to making certain that your privacy and confidentiality are protected. Your samples will be available only to researchers whose research projects have been approved by National Psoriasis Victor Henschel BioBank Oversight Committee. The National Psoriasis Victor Henschel BioBank will protect your privacy and confidentiality throughout the process, as follows:

Once we receive your completed BioBank consent form, we will assign you a unique ID number, which will be listed on your sample and medical history form. Your sample and medical history form will not contain your name or any other information that points to or identifies you. The files that link your name to your ID number will be kept in a secure location at the National Psoriasis Victor Henschel BioBank main office. The bank where your sample is stored will also have a file that links your name with your ID number. Study staff will be allowed to look at this file only after they have signed an agreement stating that they will not reveal the information to anyone else. Your name or other facts that might identify you will not appear if we present any study findings or publish results using your sample. Although all of your identifying information will be stripped from your medical history, it is still possible that the facts contained within your medical history will allow identification of you, even if the information is published anonymously.
