Here at NPF’s Patient Navigation Center, the world’s first personalized support center for individuals impacted by psoriatic disease, we served our 10,000th patient last month. Since opening in February 2016, our team of Patient Navigators has offered free and personalized guidance to help people better manage their psoriatic disease and ultimately, live a healthier life.

“The Patient Navigation Center is so important because it is the only place outside a health care provider’s office that offers answers to virtually any question that anyone could have regarding their psoriatic disease,” says dermatologist Lawrence Green, M.D. “There is no other place that has psoriasis treatment covered from A to Z with knowledgeable phone consultants just waiting to help you.” Green is an associate clinical professor at The George Washington University School of Medicine & Health Sciences and a member of NPF’s Medical Board.

The Patient Navigation Center is a core component of NPF’s five-year strategic plan to dramatically improve the health of those with psoriatic disease. By 2019, NPF aims to decrease by 50 percent the number of people who report that their condition is a problem in everyday life and increase by half the number of people receiving the right treatment. 

Assisting individuals and families

Patient Navigators are specially-trained to understand psoriatic disease and the unique issues that affect those impacted. We can help you and your family learn about the disease, access therapies recommended by your provider and connect you to others who have similar experiences. Navigators share medically-reviewed resources, help you find specialists, overcome insurance obstacles, apply to financial assistance programs and more. 

Because individuals who are more engaged with their care tend to achieve better health outcomes, Patient Navigators build lasting relationships that empower people to take control of their disease and live full, healthy lives.

Assisting health care providers 

The services provided at the Patient Navigation Center are intended to complement the care received by a health care provider. We welcome referrals from providers to help their patients navigate access issues, learn more about the disease and connect to a community of people living with psoriasis and psoriatic arthritis.

Filling a void

Psoriasis and psoriatic arthritis are complex diseases that require specialty care, treatment and lifelong management. Before the launch of the Patient Navigation Center, there were few places that people could go for personalized assistance navigating these areas. Patient Navigators fill a critical gap by providing one-on-one support to help individuals identify their personal disease challenges and overcome those barriers. Through our services, we empower our community to best manage their disease and live their healthiest lives.
