COVID-19 Event Attendance Screener

Thank you for registering for an upcoming National Psoriasis Foundation event. We look forward to you participating!

To protect your health and that of other attendees, NPF is asking all event participants, volunteers and staff to complete a health screener within 24-48 hours of the start of the event. This screener is mandatory for all participants and may be completed in advance online or on-site at the event.

During the event, all participants, volunteers and staff are expected to comply with Centers for Disease Control (CDC) guidelines, as well as state and local directives. This includes wearing masks as required, practicing social or physical distancing, using hand sanitizer and washing hands frequently, and following all signs and directions.

Please complete one screener per attendee. Parents may complete screener for minors (those under the age of 18).
