COVID-19 Event Attendance Screener

Thank you for registering for the upcoming National Psoriasis Foundation event. We look forward to you participating!

To protect your health and that of other attendees, NPF is asking all event participants, volunteers and staff to complete a health screener within 24 hours of the start of the NPF event you will attend. This screener is mandatory for all participants and must be completed online in advance.

During the event, all participants, volunteers and staff are expected to comply with Centers for Disease Control (CDC) guidelines, as well as state and local directives. This includes being fully vaccinated (unless the participant is under 12 years of age), wearing masks indoors or in crowded outdoor events, practicing social or physical distancing, using hand sanitizer, washing hands frequently, and following all signs and directions.

Please complete one screener per attendee. Parents may complete screener for minors (those under the age of 18).

If you are not fully vaccinated and are over 12 years of age:

The NPF cares about your health and that of all attendees. In accordance with NPF policy, the NPF Team respectfully directs you to remain home and not attend the event. Symptom and Exposure Questions

Please answer the following questions prior to participating in the event.







People who are fully vaccinated do NOT need to quarantine after contact with someone who had COVID-19 unless they have symptoms. However, fully vaccinated people should get tested 3-5 days after their exposure, even if they do not have symptoms, and wear a mask indoors in public at all times for 14 days following exposure or until their test result is negative. If you have questions about exposure, please contact the NPF.

This screener and event policies were reviewed by the NPF COVID-19 Task Force. This group is comprised of experts in dermatology, rheumatology, infectious disease and critical care, including members of the NPF Medical Board and SAC. The charge of the Task Force is to identify critical needs for the NPF community related to the pandemic and to serve as an expert resource to NPF in an advisory capacity. For more information on the Task Force, or the NPF response to the pandemic, please visit our COVID-19 Resource Center. Please note that it is your choice to participate and you do so at your own risk. Indoor gatherings, even with vaccination, masking and distancing policies, remain with some risk, especially for individuals who are not vaccinated or at high risk for severe COVID.
